Q: Special Note about Family Names

A: Quote from EURid regarding Family Names documentary evidence:

"Family names, unless they happen to be a company name, trade name, business identifier or any other predefined category, should be applied for in the category “Other”.
In terms of documentary evidence it is what it stated in the Sunrise rules section 12. In short that means that an affidavit signed by a competent authority, legal practioner or professional representative, is needed with a reference to a relevant law which provides protection for the name in question and proof that the applicant is the holder of that right and fulfills whatever conditions are required in that law.
This means that only sending in a copy of a passport or an identity card is not enough. Section 7 of the Sunrise rules also mentions family names and refer to section 2."
